Click on the Victim Notification Request link below. Print the form. Add your Print your registration information and sign the form. Mail the form to one of the two addresses provided on the form. PLEASE DO NOT DELAY!
We understand that some victims wish to remain anonymous due to personal safety concerns. We take seriously your need for confidentiality and we do not share victim registration information with offenders.* Without your current contact information, however, we cannot notify you about important custody changes that may affect you, such as the offender’s release from prison or placement in your community.

* If a member of the public (including the offender) requests to see registration information, a court of law will determine if your right to privacy outweighs the public right to know. The Department of Corrections will defend your right of privacy unless you waive that right. We have never received such a request but we must provide this legal disclaimer.
